Home Health Physical Therapist - Zion, IL

Join our client's exceptional Home Health team as a Physical Therapist and bring your skills directly to patients' homes. You'll assess and implement customized therapy plans to help patients improve mobility, independence, and quality of life.

Responsibilities:

  • Provide in-home PT assessments and create personalized treatment programs
  • Monitor and document progress in coordination with the care team
  • Educate patients and families on therapy routines and safe equipment use
  • Supervise PT assistants and collaborate with other providers

Schedule:

  • Monday-Friday, flexible daytime hours
  • 1 weekend every 6 weeks and 2 holidays annually

Travel: Local travel required throughout Zion and nearby areas.
