🔑 Key Responsibilities

  • Manage and deliver patient care plans

  • Collaborate with interdisciplinary care team members

  • Provide patient support in the Kansas City area

  • Ensure timely and accurate completion of charting and documentation

🕒 Schedule and Shift Details

  • Full-time hours: Monday – Friday, 8:00 AM – 5:00 PM

  • Rotating on-call nights, weekends, and 2 holidays per year
